The Nest program offers around the clock professional support tailored to meet the foster family and child's needs and help both the parents and child succeed serving children ages 4 to 9. Each child is assisted by staff with daily living skills, weekly therapy and permanency preparation activities.

What You'll Do as a Therapeutic Care Coordinator:

The Therapeutic Care Coordinator provides case management services to clients served under the Nest contract and the Behavioral Rehabilitation Services (BRS) contracts through the Oregon Department of Human Services (DHS).

  • Develop and ensure implementation of service plans and complete assessments for assigned youth and families in collaboration with Program Support Specialist (contracted mental health provider); ensure provision of daily and weekly skills building to youth in foster home and community settings based on the service plan for that youth.
  • Assess child's individual needs and coordinate outside services such as arranging and/or providing transportation to appointments, court hearings, assessment appointments, etc.; coordinate the provision of services with other agencies, responsible parties, family, foster parents etc.
  • Identify, adapt, develop ongoing evidence-based curriculum for individual and group skill building with youth in collaboration with the Program Director and Program Support Specialist.
  • Oversee implementation of the Collaborative Problem-Solving process as outlined in the child's individualized service plan in the foster home.
  • Provide role-modeling behavior, facilitate communications, and develop problem solving and conflict resolution skills for youth.
  • Provide support, coaching and training to the foster home/parents in coordination with Program Services Administrator or Program Director to ensure the youth's success in meeting their service plan goals.

Requirements:

  • Two (2) years of experience in a social services, mental health or care giving setting working with youth OR Bachelor's degree in relevant field (social work, psychology, sociology) and one (1) year relevant experience.
  • Must possess a valid driver's license from state of residence (Oregon or Washington), personal automobile liability insurance, and pass a DMV driving record check.
  • Must be able to provide proof of full Covid-19 Vaccination, may be granted religious or medical exemption with proper documentation.
  • A mask as Personal Protective Equipment (PPE) is required to perform this job safely.
